- Your surname and name
- Your contact details: private postal address or telephone number
- Your insurance – all statutory and private insurance, international insurance, self-payer information.
We do not accept: statutory accident insurance - Reason for your visit in brief
When you visit our office
- valid insurance card (eGK) for those with statutory insurance, and also for those with private insurance, if available.
- all available medical specialist findings on your case (e.g., from neurologists, radiologists, orthopaedists, surgeons, rheumatologists)
- any available prior treatment reports (discharge letters from hospital, surgery reports).
- available image material (X-rays/CT scans/MRI scans) in the form of X-ray film images, CD-ROM or in hardcopy (the latter is least suitable for diagnostic purposes). Our office does not have x-ray equipment. If new x-rays are required, we will refer you to a radiologist.
- In light of current developments, as long as social distancing restrictions are in place due to the coronavirus pandemic, we ask that you wear a face mask at all times while in our offices and that you disinfect your hands on entering and leaving the premises.
Please be advised that you may only be accompanied by another person in exceptional cases (as a parent or guardian or as an interpreter).
